A Rochester psychiatrist has been reprimanded by the Minnesota board of medical practice.

According to the order filed — Dr. Cynthia Cleveland Hauri is accused of medical practice misconduct including inappropriate prescribing of opioids for chronic pain.

She is listed as self-employed with a private practice at 300 first avenue northwest.

The order — indicates the board received a report in October of 2016 — outlining concerns of her prescribing practices that may create unnecessary danger to her patient’s life, health or safety.

In some case,s the report says the doses she prescribed were well outside of the federal drug administration guidelines.
And at least once — a controlled substance was taped outside of her office door for a patient to retrieve.

An investigation also revealed in multiple cases dr. hauri failed to document diagnosis’s, failed to recognize chemical dependency and failed to document the justification for prescribing opioids among others things.

The Minnesota board of medical practice ordered Dr. Hauri’s license be restricted and reprimanded.

That means — she is not allowed to prescribe schedule two medications — she’s required to review guidelines as issued by the federation of state medical boards and complete several courses.

She may petition for reinstatement of an unconditional license upon completion of these terms and approval from a hearing committee.
