Wailuku psychiatrist surrenders his license

Dr. John F. Balog, who specialized in psychiatry at the state Department of Health’s Maui Community Mental Health Center in Wailuku, voluntarily surrendered his license Dec. 11, the state Department of Commerce and Consumer Affairs Office of Administrative Hearings said Wednesday.
The office cited “manifest incapacity in the practice of medicine and being incompetent or engaging in multiple instances of negligence,” in a news release.
Balog allegedly failed to take accurate notes of patient meetings, properly prescribe medications for patients and properly supervise employees at the center, the news release said.
Balog surrendered his license in a Hawaii Medical Board approved settlement agreement, nixing the need for a hearing.
Attempts to contact Balog on Wednesday were unsuccessful.
State Department of Health officials did not immediately return calls for comment Wednesday.
